What is ailing my husband?

He smokes, but he walks everyday. He does like his sweets as he is diabetic, but his blood sugar is fairly good. His feet and ankles are different temperatures of coldness and quite often numb at night. Any idea what is causing this? His doctor says it is not neuropathy. So what could it be?
